REGISTER
WITH THE PRACTICE?
If you are not currently a patient of the practice, you live within our practice boundary and you wish to register, you simply need to call into the surgery with your medical card (if available). You will also be asked to provide other proof of identity and permanent address. Our receptionists will provide you with all the relevant forms that you need to complete. Separate forms must be completed for each member of your family. Your form is then considered for acceptance by the partners. We ask all new patients who register with the practice to make an appointment for a new patient health check. This enables your new GP to obtain details of your past medical history and any current needs and medications. If you have recently moved to the UK, passport identification may be required.

Patients should be aware that a charge will be incurred for visitors attending
for non-emergency healthcare.
